Caveat

  1. Changes of name may no longer be made through deed poll. The applicant, or the parent of a minor is required to contact the Registry of Births, Deaths and Marriages.

  2. The only requirement for a JP is to certify copies of documents necessary to satisfy the requirements of the Registrar of Births, Deaths and Marriages.
